OUR MISSION

The Network’s mission is to (1) strengthen and promote high quality, affordable and culturally appropriate adult day services provided by Bay Area centers; (2) increase capacity for and access to adult day services for frail elders and younger disabled adults who are ethnically and economically diverse; (3) support caregivers, and (4) participate in primary and long-term care delivery as part of the systems of care for the chronically ill.

The Network envisions a dynamic healthcare environment in which (1) adult day services are continuously expanding in order to meet the need for care generated by our growing elderly and disabled populations, (2) there is full recognition of the role and value of adult day services throughout the total community, and (3) the modalities of adult day services are vital components of an integrated primary and long-term care delivery system.

Description:
The San Francisco Adult Day Services Network is a coalition of 10 agencies in San Francisco and Marin counties at 15 locations that provide multi-disciplinary, cost-effective community-based long-term care for more than 2,500 seniors and younger adults who are nursing home eligible and may be at risk for institutional placement. Our member agencies provide substantial health, rehabilitative care and social services to assist their participants in maintaining the healthiest and most independent lifestyle possible and to reduce the stress of caregivers, who are often balancing work, family and other life commitments or personal health issues themselves. During the last year, some member agencies have increased outreach to homeless individuals and increased enrollment of those with chronic mental illnesses.

As a coalition, the Network develops Initiatives that serve our member centers as well as centers throughout the Bay Area, and also has close ties to adult day service state and national associations. The Initiatives include an Access to Care Outreach and Advocacy to improve access to adult day services, a Vision and Hearing Program to aid clients who have sensory impairments, a Quality of Care Initiative, that has produced best-practice guidelines and staff training on the most commonly seen conditions in ADHC and the Technology Initiative assisting the centers improve and expand their technology capacity. Additionally, the Network raises supplemental funding to aid low-income, private pay clients who are enrolled in our centers.

History:
The San Francisco Adult Day Services Network was created in 1983 to undertake a collaborative planning and fund-raising effort to open five adult day health programs. The new centers, together with two existing programs established by On Lok and Mt. Zion (now Goldman) Institute on Aging, made San Francisco the first city in the nation with a coordinated system of adult day health centers. A group of community leaders was convened to guide the Network’s efforts, and The United Way agreed to act as fiscal agent and to provide part-time staffing. Over the next few years, the Network raised more than $3 million dollars, opened five centers, and established a coordinated transportation system.

The Network’s purpose includes service development, government relations, and multi-agency planning. In addition, the Network seeks to increase public awareness of adult day services through a variety of marketing and advocacy efforts. The Network also places great value on community relations and has a close working relationship with city and county departments.

In 1990, the Network received the U.S. Secretary of Health and Human Services’ Community Health Promotion Award and the U.S. Administration on Aging Community Achievement Award, which included a grant to assist California communities to develop similar networks. In 1992, the Network gave a presentation on its collaborative approach at a conference in Washington, D.C. In 1994, the Network incorporated as a 501(c)(3) nonprofit corporation. Network Board members served as California delegates to the 1995 White House Conference on Aging. Today, volunteers and staff members serve leadership roles locally and on the state and national levels.
