The Emergency Department Case Management Program is a proven, nationally recognized program that provides long-term comprehensive psychiatric, psychosocial, and medical care for underserved individuals who are medically fragile and often homeless who frequently rely on medical emergency services at San Francisco General Hospital.

Description:
An interdisciplinary team of social work case managers, nurse practitioner, primary care physician and psychiatrist provide medical and mental health counseling, intensive case management and crisis intervention services to people who frequently rely on medical emergency services at SFGH. Clients often have a complex interplay of medical, emotional substance abuse and psychosocial problems.
Our interdisciplinary team employs creative, flexible, and nonjudgmental approaches for minimizing harm and improving the quality of life for our clients while reducing overall health care costs.


History:
EDCM was established 1995.
The program has served as a model for similar programs being developed throughout the state and nationally. EDCM has been awarded the Safety Net Award by the National Association of Public Health Hospitals (1998)the Best Practice Award by the California Association of Public Health Hospitals (1998)and was named as one of five Best Practices programs (2002)by the California Endowment and California Health Care Foundation.
