The mission of Self-Help Home Care & Hospice is to provide quality home health and hospice services, information, and education to the Chinese community; to assist the patient and their family in optimizing their quality of life; and to advocate for the patient in accessing health care services.

Description:
Self-Help Home Care & Hospice is a program of Self-Help for the Elderly, a multi-service agency. The agency provides a range of health and social services that include senior centers, employment training, nutrition programs, social services, housing, adult day services, home health care, and hospice.

History:
Self-Help was founded in 1966 to meet the needs of isolated Chinese elderly. Since then the agency has grown to become a multi-service agency. The Home Care program was started in 1983.
