The name On Lok means "place of peace and happiness" in Cantonese, and this reflects both our roots and our philosophy of care. Through five related not-for-profit entities, On Lok provides healthcare, housing and intergenerational programs that meet the needs of seniors. The largest and oldest component of On Lok's services is On Lok Senior Health Services, whose mission is to provide quality, affordable care services for the well-being of the frail elderly. We do this through On Lok Senior Health, a fully-integrated and licensed health plan that delivers medical and long-term care to those looking for an alternative to a nursing home. At present, On Lok Senior Health helps about 950 elder adults maintain their independence - by providing all necessary primary and specialty medical care, adult day health care, in-home health and personal care, social work services and hospital and nursing home care. Our vision is to continually lead the movement to improve care for the elderly.

Description:
On Lok Senior Health, a Program of All-inclusive Care for the Elderly (PACE) program, is specifically designed to provide total long-term care for the frail elderly and provides an important alternative to nursing home care and placement. The comprehensive health plan includes full medical care, prescription drugs, home care, adult day health, transportation, and more, allowing participants to remain at home for as long as possible. Participants voluntarily join the health plan and receive all their health and medical care through it. We have centers located in San Francisco and Fremont, CA. Our centers in San Francisco are located at: 1333 Bush, 1426 Fillmore Street, 2700 Geary Street, 1000 Montgomery Street, 4430 Mission Street, and 1441 Powell Street. In Fremont, CA our site is located at 159 Washington Boulevard.

History:
Serving the elderly for over 35 years, On Lok has grown from its humble roots as a senior day healthcare center to a complete medical and health-related services program that is now replicated throughout the United States.
