The Mission of Pathways Home Health Hospice and Private Duty is to provide compassionate high quality patient and family centered home health and hospice care, promoting comfort, independence, and dignity.

Description:
Pathways, a local, non-profit healthcare agency servicing 5 Bay Area counties, is committed to serving patients and their families during their end of life process. It prides itself in providing patient centered care in a team atmosphere combining the skills of doctors, nurses, social workers, spiritual care specialists, volunteers, and families.
Pathways utilizes volunteers in nearly all aspects of the organization. Many volunteer provide companionship to patients and caregiver relief to tired family members. Other provide adminsitrative support in our offices so that we may provide care to our patients. Volunteers also service family members after a patient has passed away through bereavement support - including telephone calls, grief groups and individual counseling. We also have volunteers assist with fundraising and community awareness.
